There sis a bug happening to my friend (but never me) where he cannot pick up loot in an area about twice large as the character's height (even when no players or enemies are nearby covering the loot.

I have this happen and the work around I found is to right click something in your inventory and then left click the same item, It seems to be a fix. At least to me it's a fix.
