Problems with Scaling of difficulty or am i lame ?

Hi everyone, first of all i hope i'm at the right place to create this thread.
Then, i'd like to share with you guys my feedback about the scaling of difficulty, i have a melee (lvl 47 spin2win build) and a mage (lvl 28 plaguelord build that i'm levelling up ATM with Bounties and rifts) :
I don't eff understand why sometimes when i do bounties and/or rifts i'm like a potatoe crisp. Let me get to the point :
From levels 1 to 20 the bounties/rifts were ridiculously easy to do. Just after i reached lvl 20 with my mage for example, these mods gave me the impressions the difficulties were totally phucked up, especially with blues and yellows mobs.
My questions :
- Are the mobs scaled at a difficulty 10 levels more than a toon ? For example : if a toon is level 30, is an elite (blue and yellow) scaled at a level 40 ?
- Why i do have the impression the second level of a rift is harder than the first level ?
- Why sometimes i can clear the first level of a rift and when i start a new one i get killed at 25% of completion ?

To understand what i try to explain, i give you some stats :
Melee Spin2Win : 577 ferocity, 44.7% of Crit Chance on attacks, 318% of bonuses based on attributes, 265,5% ferocity bonus
Chance to inflict bleed : 16.6%
The gems with my gear :
Helm : +200 HP
Cuirass : +250 HP +200 HP +13% score on Physical damages
Belt : +18% chance to bleed with rend
Legs : +200 HP +12% Physical resistance
Weapon 2H : +10% Speed attack and incantation +12% Global Critical Damages (have Material Afflictions Physical and Rend)

FIY : I gemmed a speed attack gem because more than my speed attack is high, more than i give critical damages :)

So if someone can help me understand if i do things wrong, i'm all ears.
