Why can I not save a game, exit it, and come back into the very same spot? No, it has to be the last major milestone area. Now, I was deep into the brotherhood, and beating MERCY, and I can't get back to where it's next.

Well, games like this are procedural, the map is randomly generated....every time. You're supposed to exit the game in town...as with EVERY Aprg, you wanna exit, in town. (You can't be loaded into the exact same spot if the game is randomly generating the area when you log back in)
