This bug has been happening a long time with any skill you have to manually aim: other examples include Flight of Gavinir and Anvil's woe.
My observation is that while on your own screen your character looks fine, your co-op friends (and perhaps the server as well?) will see your character locked in a certain direction skating around as you move without rotating at all.
In practice this means any attack or projectile you fire off will look correct on your own screen, but in reality they will be firing off in some fixed direction giving the appearance of not damaging enemies because the animations on your own screen are incorrect.
For example if one figured out that their character was locked facing the 6 o'clock direction, you could click an anvil's woe directly north of you (12 o'clock) but see the damage numbers register on enemies directly south of your character even though the animations wont appear to line up at all.
This bug or server issue - whatever it is - has existed since before the bloodtrail update and is completely game breaking for on-line co op for any build requiring precision aiming or inputs.
