Sometimes when I cancel my primordial form (Aspect of Dawn), my character just straight up dies because my health is set to 0 after it ends.
Doesn't matter if I'm in combat or not, doesn't matter if there's no enemies anywhere nearby. Just kills my character.
Sometimes, but not always.
It used to set me down to 25% health before. Now it's just straight up 0.

I understand the bug a bit better now.
It appears that going into primordial form somehow cancels out most of your stats, and sometimes the stats arent counted properly for me to begin with, so my health is set to negative numbers.
My max health variates greatly from game to game, sometimes 36000~ currentl it's sitting at 41455, when I go into primordial form right now it goes down to 8000 instead of killing me.
And when I cancel primordial form, the damage of the primordial abilities goes up by 1000%
For example, my "Hammer of the Aurora" ticks for around 3500 every second or so on all targets, but as soon as I cancel the primordial form, it ticks for 35000.
