When you are playing with Feeding Swarm and you die, but there is one zombie still alive. You can not revive, because an allie is still in combat.
Need to leave the game and start again.

This happened to me when I didn't even have Feeding Swarm, or anything that results in allies. It was just me and my sword, I died, and could not revive because of the ally still in combat.
I was playing a character that starts with sword, and the only other skill I'd unlocked was Wings of Ishtar. It was fighting a monster in Act 1.
