I'm french i'm sorry if i do mistake.
I have a build with counter attack when i block and i do insane damage but i don't know how. Maybe it's bug or maybe it's normal? With the description on the counter on block i'm not suppose do so much.
I do a video but my skill tree and gear description are in french.
When i play i have a stream, we can hear the sound sorry. :x

The other bug is with the flesh transformation, when a monster is out of the level, you can teleport under ground if you aim him exactly with your cursor.
The result is in the video, you arrive in hell maybe? x)
