This has happened several times in the past couple of weeks where I'll be fighting enemies and suddenly sent to the title screen. This usually happens in the 3rd stage of a portal quest. It doesn't happen every game, maybe every 2nd or 3rd.

Don't use plagueburst at all, this should fix many problems you might have :)
