I seem to have noticed these occur when opening a chest. I am in combat and I'm using Trial Belt. Has happened with two different builds. One for bleeding edge (current), and a summoner build using anomaly. To reproduce, fight a pack of trash, open chest, disconnect. Seems to happen more on second level. In the few times I tried yesterday I could not complete a single turn-in because the game would just disconnect and I would lose all progress! In each case there would be a 20 second or so pause, then I would return to the character screen.
