i've taken all gear that adds x-y to attacks off, and it will never give an ailment debuff to any enemy... all my bullets do physical damage from what i can tell...
i have the Regicide mark III (pure phys gun) i have over 30% chance to apply ailment (and i've shot over a hundred bullets for testing) not once have i applied any ailment.
