I entered a side are while in the "Necropolis".
Got the sidequest "Cull All Evil": Kill enemies in the area.
Clearing the entire area only filled up two thirds of the bar.
The area was really small.

I just experienced this too... this has occurred in on-line mode only. I played to act two last night (in offline mode)and had several of these complete without issue.
