Offline Mode
Noticed that the graphical icon for my "W" potion slot was missing. If I highlight the area it shows the potion, but doesn't seem to work.
I replaced it with an upgraded version at some point and it seems to have fixed the issue.

I've noticed while starting a new act, that it removes the option to use potions, and it appears as if they were removed, but if you put a new one in the slot it suddenly pops into the inventory, being able to switch them back and forth.
I keep 1 potion in my stash just to fix this issue.
