On the fight with the Cultist of the Sleeper; he's twice now teleported me outside of the boss 'boundaries' so when I defeat him I am unable to go in to talk to the Primordial Guardian. I've had to twice log out and log back in, only to find that the game didn't record the kill.

Anyways I made sure not to get caught by that teleport ability at all the 3rd time. Just bringing this to attention; if that's part of the mechanic at least have the boundary gate comedown afterwards. Also would those two successful kills that didn't get recorded because I had to logout to get unstuck count as a death (I'm working towards the no dying in chapter 1 achievement)?
