If you activate a Upgrade Rarity project, and either click "cancel" or try and using a different item and activating, the previous item put in will disappear, along with the gold/affinity needed to activate the project. This happened to me online. IGN Rayhn

I just had sort of the same issue, I started the quest then logged off. I came back teh next day, completed the quest and the item disappeared... wtf
