I've been having an issue with Bladestorm. It appears that if I use it too soon after using Blood for Blood that it will not hit any enemies at all. I'll admit that I haven't looked at the forums too hard to find if it has been addressed in any capacity, but what I have looked through I haven't seen anything about it.

It's not a Bladestorm bug, it happens to all skills fast enough to act immediately after another skill's use. Try using Bleeding Edge immediately after something like Wings. That first axe will spin but hit nothing. It's just more pronounced with Bladestorm because it lasts longer.
